Sana Benefits leases the Prime Health Services Practitioner and Ancillary Only network so that Sana members can access all providers in this network. The full list of participating Prime Health network affiliates and network logos can be found here.
If your practice is in-network, Sana will pay claims at the Prime Health contracted rate. If you have questions about your Prime Health contract or rates, please contact the Prime Health team directly at (877) 277-4635.
Please send claims to Sana directly using the address and electronic payer ID listed on the back of the Sana member ID card.
