If you're receiving care for certain conditions and are actively working with a care team when your Sana First coverage starts, you may be able to keep seeing your current provider instead of starting over. This is called Transition of Care. You'll need to request it within 30 calendar days of your Sana First plan start date.
Normally, care under Sana First works like this: any referrals, prior authorizations, or approvals from a previous plan don't carry over automatically. Once your Sana First coverage starts, any care you receive—including from a provider you're already seeing—needs an active referral from your Sana Care team. Transition of Care allows you to see your same provider, if you qualify.
You may qualify for Transition of Care if you are currently:
- Actively being treated for a serious or complex condition—for example, ongoing chemotherapy or radiation, an organ transplant evaluation, or recovering from a recent stroke
- Scheduled for a non-elective surgery, or recovering from one, that was already scheduled and approved before your Sana First plan start date
- Receiving palliative or end-of-life care
- Admitted to a hospital, rehabilitation facility, or skilled nursing facility on your Sana First plan start date
Transition of Care is not available for routine, elective, or preventive care.
To request Transition of Care:
- Log in to your Sana member portal and navigate to the Sana Care tab on the left.
- From there, select “Get care now” to message the Sana Care team. Let them know your care situation and that you are requesting a Transition of Care. This must be done within 30 days of your Sana First plan start date. Requests submitted after that will be denied, and standard referral rules will apply going forward.
- Sana Care will follow up with next steps—including asking for documentation showing you were actively being treated by your provider in the 30 days leading up to your Sana First plan start date. It's worth having this ready, since it's needed to complete your request within that same 30-day window.
If your provider is not contracted with Sana: qualifying for Transition of Care doesn't guarantee approval on its own—if your provider is not contracted with Sana, they will have to agree to Sana's payment and administrative terms. If they're not willing to, Sana Care will help you find a qualified alternative so your care isn't interrupted.
