Chiropractic Care: Costs, Coverage, and Limits

Chiropractic coverage depends on your plan's effective date.

Plans effective before 9/1/26
Chiropractic Care is covered for qualifying members:

• $25 copay for qualifying members on PPO Plus plans
• Covered and subject to the deductible for qualifying members of PPO Plus HSA plans
• No referral or prior authorization required
• Limited to 30 sessions per year, per member

Chiropractic Care may include spinal adjustment and manipulation, and other modalities performed by a physician or other licensed practitioner such as a doctor of chiropractic (DC).

Plans effective on or after 9/1/26
Chiropractic Care is not a covered benefit. Members will be responsible for the full cost of chiropractor services.
