How to Find and Use Your Summary Plan Description (SPD)

Your Summary Plan Description (SPD) is the official legal document detailing how your health plan operates. It outlines covered benefits, exclusions, cost-sharing percentages, and specific plan rules.

How to Locate Your SPD in the Member Portal

  1. Log in to your Sana Member Portal.
  2. On the left-hand navigation menu under Resources, click Documents.
  3. Under the Summary of Benefits and Coverage section, locate Sana Health Plan.
  4. Select your active plan type and date to view your enrollment details.
  5. Click Summary Plan Description to open the document PDF.

Searching and Reading Your SPD

1. Quick Search Methods

  • Table of Contents: Browse major section headers to jump to general categories (e.g., Inpatient Care, Prescription Drugs, or General Exclusions).
  • Keyword Search (Cmd+F or Ctrl+F): Press Cmd + F (Mac) or Ctrl + F (PC) to open the search bar. Enter specific terms such as "physical therapy," "chiropractic," or "durable medical equipment."

2. What Each Search Result Tells You

Once you locate a specific procedure or benefit section, your SPD provides:

  • Service Definition: A detailed description of what the medical procedure or treatment entails.
  • Coverage Status: Clear confirmation of whether the service is Covered, Not Covered (Excluded), or requires Prior Authorization.
  • Cost-Sharing Percentage: The percentage share paid by the plan versus your out-of-pocket obligation (e.g., 80% plan coverage after deductible).
  • Limitations & Comments: Any specific conditions, such as annual visit caps (e.g., 20 visits per calendar year) or pre-certification requirements.
