Plans effective before 9/1/26
Hearing aids are covered under coinsurance after the deductible is met. Benefits are limited to one pair every three years with Prior Authorization (PA), plus maintenance and repair.
Plans effective on or after 9/1/26
Hearing aids are not a covered benefit. Members will be responsible for the full cost of hearing aids, including maintenance and repair. Amounts paid out of pocket do not count toward your deductible or out-of-pocket maximum.
