Existing Contracts: Add New Locations or Providers

To add additional locations or providers to an existing contract, please:

  1. Make a copy of this roster template (download and complete on your computer)
  2. Save it as a .CSV file
  3. Email it to contracting@sanabenefits.com

We'll get started and reach out with any questions.

Provider Directory Update Timing
Our provider directory is updated once per month, generally within the first 10 days of the month. This update schedule reflects when information is published to the directory, not when a provider or location becomes contractually effective.
