Your estimate will include any copays, deductibles, or coinsurance associated with your upcoming procedure. Below are a few considerations to keep in mind.

An estimate of cost-sharing liability does not guarantee coverage. 

Your Summary Plan Description (SPD) offers a complete summary of your benefits, limitations, and other plan provisions. Look for your SPD in the Document Center of your Sana account.

Services billed may vary depending on the care you receive. 

Before scheduling your procedure, we recommend asking your provider for a good-faith estimate of expected charges to identify all care required for the procedure, which may include the following:

    • Labs
    • Facility fees
    • Anesthesia
    • Pre-or post-surgical therapy
    • Etc.

Actual charges may vary from your estimate if:

    • Your plan changes
    • You have additional claims or year-to-date spending prior to the procedure
    • Your provider offers different services due to complications, secondary conditions, or other unknown factors during the procedure
    • Services billed as preventive may be no-cost

Prior Authorization

Your provider's office will generally coordinate any requirements related to prior authorizations and visit or treatment limits.

Balance Bills

Our network of partners includes a variety of modern and accessible providers who are available for everything from telehealth to complex procedures. We are happy to work with any provider, and we'll never charge out-of-network fees.

To avoid the rare inconvenience (less than 3%) of Balance Bills from non-contracted providers, we recommend using our Find Care tool to find an in-network Care Partner, Contracted Provider, or Affiliate Network Provider.
