How to Request a Report of Your Claims and Diagnostic Codes

You have the right to request a copy of your medical claims and diagnostic (Dx) codes from Sana at any time. This information can be helpful if you're reviewing your healthcare history or working with a broker to apply for secondary coverage.

Note: You will still receive monthly Explanation of Benefits (EOBs) to the address on the subscriber's account and your claims information is available at any time from the member portal. Claims and Dx Reports should only be requested if the paper EOBs and information available in the portal do not satisfy your need. 

What’s included in the report?

Your report includes detailed information about your medical and pharmacy claims, such as:

  • Claim details: Claim number, type (medical or pharmacy), and date of service
  • Provider information: The name of the provider who submitted the claim
  • Diagnosis codes: Both primary and any additional diagnosis (Dx) codes submitted by your provider
  • Financial breakdown:
    • Billed amount
    • Amount covered by your plan
    • Copay, coinsurance, and deductible amounts
    • Any non-covered charges or plan savings

Protecting Your Health Information

This report contains Protected Health Information (PHI). To protect your privacy and comply with HIPAA:

  • Only you (the member) can request this information.
  • If someone else (like a broker, admin, or family member) is requesting a copy, you’ll need to submit a Medical Release Form or have a HIPAA-compliant authorization on file with Sana prior to us sending a Claims and Dx report to anyone but you.

Once you receive your report, you can choose whether to share it with others. Keep in mind that sending it to a third party means you are authorizing them to view your personal health data.

 

How to Request Your Claims & Dx Code Report

To request your report, contact our Member Support team via our email address: hello@sanabenefits.com. Please include the following information:

  • Your Member ID (found on your ID card)
  • Your Full Name (as it appears on on your ID Card) 
  • Your Date of Birth 
  • The date range for the claims you’re requesting (mm/dd/yyyy – mm/dd/yyyy) 

We'll prepare your report and send it to you securely, typically within 1–2 business days.
