You can easily locate your specific premium costs and tier pricing using the options below.

Option 1: Contact Your HR Manager or Payroll Department

Because your employer manages payroll deductions directly, your internal Human Resources admin or payroll team can confirm your exact per-paycheck contribution, tax setup (pre-tax vs. post-tax), and elected coverage tier.

Option 2: Locate Your Employer Benefits Guide in the Portal

Your specific plan offerings and tier pricing (Employee Only, Employee + Children, Employee + Spouse, or Employee + Family) are detailed in your company's official Benefits Guide.

To access this document:

  1. Log in to your Sana Member Portal.
  2. Navigate to the Documents section.
  3. Scroll down to the Plan Information section at the bottom of the page.
  4. Locate and download the document titled [Company Name] - Benefits Guide - MM/DD/YYYY.
  5. Open the guide to view all available plan options and their corresponding premium costs for each coverage tier.

Note: While your Summary Plan Description (SPD) and Summary of Benefits & Coverage (SBC) are also located in the Plan Information section, those documents detail copays, deductibles, and covered services rather than your payroll deduction amounts.
