How to Cancel Your Sana Health Coverage

Whether you are gaining coverage through a new employer, joining a spouse's plan, or experiencing another life transition, here is what you need to know about canceling your coverage with Sana.

Canceling Coverage Mid-Year (Qualifying Life Events)

Because health plans operate on annual contracts, members generally can only cancel coverage mid-year if they experience a Qualifying Life Event (QLE), such as:

  • Gaining coverage through a spouse or domestic partner's plan
  • Enrolling in coverage with a new employer
  • Becoming eligible for Medicare or TRICARE

Important: QLE requests must be submitted within 30 days of the event date. You will be required to provide proof of your new coverage showing the new effective date.

How to Submit a Cancellation Request

  1. Notify Your Employer: As a primary subscriber, contact your employer's HR or Benefits Administrator first, as mid-year plan changes must be processed through your company's payroll/benefits system.
  2. Submit via Sana: If instructed by your HR team or if you manage your account directly, click the Chat Bot icon in the bottom-right corner of your screen to request a cancellation.
  3. Provide Required Information:
    • Full Name & Member ID
    • Reason for cancellation
    • Requested effective date of cancellation
    • Proof of new coverage (if canceling mid-year due to a QLE)

Important Things to Keep in Mind

  • Effective Dates: Coverage cancellations typically take effect on the last day of the month in which the request/QLE occurred, unless otherwise governed by your employer's plan policy.
  • Canceling Dependents Only: You can choose to cancel coverage for specific dependents without canceling your own primary coverage, provided a qualifying event applies.
