How to Request a Certificate of Coverage

A Certificate of Coverage is an official document verifying that you or your dependents are (or were) enrolled in a Sana medical plan.

When You Might Need One

You may be asked to provide proof of coverage in scenarios such as:

  • Canceling Previous Insurance: Proving new coverage after enrolling with Sana during a Qualifying Life Event (QLE).
  • University Health Plan Waivers: Demonstrating that a college student dependent has primary coverage under your plan.
  • Legal & Official Documentation: Submitting proof of coverage for immigration, U.S. residency, or court applications.

How to Request Your Certificate

You can request a Certificate of Coverage directly through our support team:

  1. Open Support Chat: Click the Chat Bot icon in the bottom-right corner of your screen.
  2. Submit Your Request: Type "Requesting a Certificate of Coverage."
  3. Provide Required Details: Ensure you have the following information ready for yourself and any covered dependents needing proof:
    • Full Legal Name
    • Date of Birth
    • Sana Member ID Number

Once submitted, our team will generate your document and email a PDF copy directly to the primary account holder.
