Are you on a Sana First Health Plan (SFHP)?
Finding a provider works a little differently with Sana First.
For most in-person care, adult Sana First members must start with Sana Care before seeing an outside provider or specialist. Sana Care will evaluate your needs and, when in-person care is needed, connect you with Care Navigation to help find provider options and complete the referral process.
If you receive care that requires a referral without first completing the Sana Care referral process, the claim will be denied and you'll be responsible for 100% of the cost.
Sana First is not an HMO or EPO with a fixed provider directory. Instead, Care Navigation provides a guided path to care and can help identify options through Sana's direct contracts, HealthSmart, or direct-bill providers based on your medical needs, location, quality, availability, and cost.
Finding Care for Children 17 and Under
Pediatric primary and preventive care does not require a Sana Care referral.
If your child's pediatrician recommends specialty care, submit a copy of the pediatrician's referral or after-visit summary to Care Navigation through your child's profile. Care Navigation will help coordinate the appropriate referral and next steps.

- Explore top search categories and Care Partners, or search by provider name or specialty
Your search results may include:
- Care Partners: Best-in-class providers offering high-quality care, often at no-cost for PPO Plus members or either no-cost or free after deductible for PPO Plus HSA members, depending on care type and current IRS regulations for HSA members
- Contracted Providers: Practices that already recognize and accept Sana

Can’t Find What You’re Looking For?
Sana accepts claims from out-of-network providers without any penalties. If your preferred provider isn't listed, they can still bill Sana directly. Show them your ID card to confirm benefits.
